General wellness use

Onset Clinic Portal is a general wellness organization and information tool. It helps your clinic organize supplement routines, fitness templates, and patient-shared wellness data. It does not provide medical advice, diagnoses, or treatment recommendations.

Not a software medical device

Onset is not a software as a medical device (SaMD) and is not FDA-cleared or FDA-approved for diagnosis, treatment, or clinical decision support. Do not use the portal as a substitute for your clinical judgment or regulated medical records systems.

SHIELD security posture

Onset uses SHIELD-style safeguards — patient consent controls, audit visibility, encryption, and least-privilege access — designed to exceed typical HIPAA technical safeguard expectations. Onset is not acting as your HIPAA-covered entity or business associate EHR unless you have a separate signed agreement with Last 1 Enterprises LLC.

Your recordkeeping responsibility

Your clinic remains solely responsible for patient medical records, legal record retention, informed consent, and compliance with applicable laws. Export patient directory data from Settings before closing a clinic account if you need a local copy for your records.

Data deletion

Deleting a patient relationship or closing your clinic removes clinic-held copies of assignments and patient-shared data from Onset servers. It does not delete a patient's personal Onset app account or data on their device. Closing a clinic permanently deletes all patient directory and clinic protocol data stored in this portal for your organization.
